New Enrollment
Please review the Enrollment Checklist for information regarding registration of new students. Specific forms for students who are enrolling with District 67 for the first time are NOT available online. You must contact your school or the district office (summer only) for these forms. To download a registration/fee packet or invoice, click on the links below.
Continuing Students
The District 67 Fee Packet for the 2011-2012 school year was mailed to each registered student, and included the instruction letter, student registration (material fee) invoice, Extended Day Kindergarten Invoice (if applicable) and bus registration. - Material Fee Invoice (Required) (payable to Lake Forest School District 67).
- Extended Day Kindergarten Tuition Invoice (Required) – if applicable.
- Bus Registration (Optional) (payable to Lake Forest School District 67) – Please return one form per family.
- Bus Behavior Agreement Form (Required) - Download and return one form per child.
- APT Student Directory Update (Optional) - Please download and return one form per child if new family or your information has changed.
- Spirit of 67 Donation Form (Optional) (payable to Spirit of 67 Foundation) - Please download and return.
